Prices below do not include medications or labs- those will be charged in addition if ordered. PAYMENT POLICIES

• This is a direct-pay practice

• We keep costs low by having no billing or collections staff, therefore EVERYONE must pay their FULL BILL directly TODAY

• Cash, checks, credit and debit cards are accepted

• Writers of returned checks will be charged a $35 processing fee

• Failure to pay your entire bill at the time of service will incur a $35 accounts receivable management fee. Future office visits will be required to be PRE-PAID (cash/credit/debit card only – no checks) BEFORE another appointment will be scheduled

• We will provide an itemized receipt that you may use for insurance purposes if you wish

• Medicare/Medical Assistance/BadgerCare patients, by law, may NOT seek reimbursement from insurance for care provided at Doctors Care Medical Clinic.

• Failure to cancel an appointment for which you do not show up will require you to PREPAY (cash/credit/debit card only) your next office visit before another appointment is scheduled

Our availability is subject to first come first serve and is not guaranteed, however, our goal is to get back to you within the business day. We will schedule a time when we are able to call you back. We reserve the right to require an in-office visit depending on the severity of the illness before giving our professional option for treatment.
