*Additional expense may be incurred for supplies-pathology fee, injectable medication ($10-20), casting ($40-80) pap smears, lab work (varies), skin biopsies ($75), laceration repair ($30), incision and drainage of abscess ($ 30), toenail removal ($30), house calls (<10 miles $50, 10-20 miles $100 etc. Any added costs are always discussed with patients prior to ordering any tests or treatments.

Children age 4-17: $29
must include an adult enrolled as a member

Adults age 18-39: $49

Adults age 40-64: $69

Adults age 65+: $89

Family Plan

Single parent family & unlimited children in same household: $160/month

Two parent family & unlimited children in same household: $200/month

Businesses

Each employee: $59/5 or more employees enrolled
